Rachel is 24, has a severe learning disability, epilepsy, cerebral palsy and a history of psychosis.
She had been yo-yoing between hospitals and residential care since leaving her specialist residential college at 18 and had recently begun scratching people who approached her and self-harming by banging her head against her bedroom wall.
Rachel needs high levels of support with most aspects of daily life and is partially sighted.
Rachel’s social worker approached us to put together a package that could support her to live a better life.
Through our detailed assessments we identified great potential to provide Rachel with a better quality of life supported by a staff team that she trusted. The NobleCare team devised a person-centred plan that included her sensory needs and intensive interaction through our Effective Care programme. Specialist input was offered by our consultant behavioural specialist. These approaches helped develop reciprocal interaction by Rachel and enabled her to begin to participate in activities and relationships.
Rachel recently raised £300 on red nose day and has planned a holiday to go to Cornwall.
