Rachel is 24, has a severe learning disability, epilepsy, cerebral palsy and a history of psychosis.

She had been yo-yoing between hospitals and residential care since leaving her specialist residential college at 18 and had recently begun scratching people who approached her and self-harming by banging her head against her bedroom wall.

Rachel needs high levels of support with most aspects of daily life and is partially sighted.

Rachel’s social worker approached us to put together a package that could support her to live a better life.
